Child and Family Team Coordination
The Partners’ family decision-making model wraps a team of professionals, friends and extended family members around each family affected, to assist them in making decisions on an individualized strength-based service plan.
Families are assigned to a Child and Family Team Coordinator who facilitates a team of professionals and family members to participate in service plan development and service delivery strategies for the family. Ongoing meetings may cover a review of treatment goals, progress made, problem-solving, new goal development and continued education for all team members.
Our goal is to address all facets of a family's needs; an approach that empowers families to make their own decisions about the safety and care of their children. Partners’ treatment model was identified in 2010, as a Promising Practice by SAMHSA’s National Registry of Evidence Based Programs & Practices with the state of Washington.
